Post-Secondary Transition


By age 14, the Ohio IEP must include a Transition Plan for all students with disabilities which documents:

  • A thoughtfully considered Future Planning Statement based on discussions with the child and the child’s family. This statement will include plans for the child’s future including school and life beyond school. It will consider future education/training, employment and independent living options, summarizes the child’s skills, preferences, interests, needs and strengths. This Future Planning Statement is the foundation for rich transition planning.
  • Focused, annually updated Age Appropriate Transition Assessments providing evidence the student is on the right track to meet their future plan, and revealing the need for activities and services to close gaps while providing data for the development of annual IEP goals.
  • Annually updated Post-Secondary Measurable Goals written for the future, after a student graduates, in the areas of Education and Training, Employment and Independent Living, if appropriate.
  • Student is taking a Course of Study that supports their Post-Secondary Measurable Goals, and the pathway to graduation is considered.
  • The district is providing Transition Services and Activities that help close the gap toward meeting Post-Secondary measurable Goals as revealed in the Age Appropriate Transition Assessments.
  • The student is invited to the IEP team meeting to participate in their transition planning process.
  • Adult service agencies are invited to the IEP if the student is eligible.
