By attending and signing the sign-in sheet, I willingly grant my permission to be photographed, video taped, or otherwise recorded. My image and statements may be used in publications, which include but are not limited to, the State Support Team Region 2 website, monthly newsletters, brochures and other publications.
I transfer ownership of my participation including all author's rights, copyrights, or other claims that I may have to the State Support Team Region 2, 1885 Lake Avenue, Elyria, OH 44035, 440-324-5777.
I release State Support Team Region 2 administrators and personnel employed or contracted by the State Support Team from any and all liability arising out of my participation or inclusion in publications, productions and programs which may be produced.
I do not expect, nor will I receive, financial remuneration, royalities or compensation of any kind for any photos or videos of my participation or appearance. I fully understand that neither I nor my heirs or assigned representatives have any claims whatsoever concerning my appearance and participation.
I understand that it is my responsibility to notify a State Support Team Region 2 staff member immediately in writing if I do not wish to be photographed or video taped.
